Get the all-clear first
Most mums with GDM are encouraged to start hand-expressing from around 36–37 weeks, but every pregnancy is different. A quick check-in with your midwife, GP, or diabetes educator will confirm the safest timing for you.

On insulin?
Some care providers tweak their recommendations for antenatal expression based on insulin use. If you're using insulin, it’s worth asking whether they’d like you to start earlier, later, or stick to the standard guideline.

Not right for everyone
Conditions like a history of preterm labour, placenta complications, or carrying multiples may mean antenatal expressing isn’t recommended. Your care team can let you know whether it’s appropriate for you.
Why express colostrum?
Colostrum is called liquid gold for a reason. It’s incredibly concentrated and packed with antibodies, immune cells and energy-rich nutrients. Even a few drops can offer meaningful protection for your baby’s gut, support their immune system and provide quick, gentle fuel as they adjust after birth.

Why early feeds matter for GDM babies
Babies born to mums with gestational diabetes can be more prone to low blood sugar in the hours after birth. If your baby needs extra support, staff may suggest formula or glucose gel while your milk is still coming in.
